Where is the patient/client data managed - at the care faciility or at the doctor’s virtual office?

The patients or residents to whom teleconsultation can be initiated are basically managed by the "owner of the private cloud" (i.e. our contractor / client).

This results in free cases:

1. Care facility is our customer, and not the physician

The supervising family doctors get a "guest access" with their own login / password to the home Cloud via the admin of the care facility. This allows the access to care facility residents who are also patients of the respective primary care physician, but not to the other residents. The patient data is maintained by the nursing facility, or via an existing interface to the nursing documentation system. The facility's resident IDs are used for this purpose.

The consultation is initiated by the facility care and completed by the physician. At the end of the teleconsultation, a report that can be filed on both sides (as a PDF) is generated. For the nursing facility, there is also the possibility to import the data into the nursing documentation software (if the interface is already implemented).


2. Physician is our customer, care facility is not

The nursing facility gets a "guest access" with its own login / password to the Practice Cloud via the admin of the doctor's practice - also restricted to its own clients. The patient data is maintained by the medical practice, if necessary, via an existing interface to the practice information system. Patient IDs from the practice software are used for this purpose.

The consultation is initiated by the home care and completed by the physician. At the end of the videoconsultation, a report that can be filed on both sides (as a PDF) is generated. For the doctor's office, it is also possible to import the data into the practice information software (if the interface is already implemented). This is particularly easy with Duria but, in principle, also with other systems via GDT.


3. Physician is customer with own cloud, care facility as well

To carry out a teleconsultation, both partners must be in the same cloud, i.e. either in the practice’s or in the home’s cloud (one of them as a guest of the other). To do this, one can select the cloud in the TeleDoc software in the login screen (similar to Microsoft Teams). If ‘doctor’ has an interface connection to the practice information software, then it makes sense, one works primarily in the practice’s cloud (for the home this creates no disadvantage, but for the doctor an advantage). If ‘home’ has an interface connection to the nursing documentation software, then one works primarily in the home’s cloud.

We are currently working on a good solution for the case of both partners having an interface to the documentation system. However, we have not yet had this case with any customers. The solution is to merge the two patient or client IDs at our site and assign them a unique "Master Patient Index". Then, both interfaces can be operated, and the reports assigned uniquely in each case.
